How it Works

  • If you or your loved one are not already on a waiver, request a MNCHOICES assessment from the county in which the person lives. The MNCHOICES assessment will determine the CDCS budget.

  • Let your MNCHOICES assessor and/or Case Manager know you would like to start CDCS.

  • Choose a Financial Management Services (FMS) provider to manage the budget. They will also manage hiring and payroll if you choose to have support staff.

  • Select a Support Planner to help you understand your options, write your Community Support Plan (CSP) , and submit the plan to your Case Manager for review.

  • Your Case Manager will let you know if the plan is approved and will send the final plan to your team.

  • Contact your support planner as needed throughout the service span with any plan changes or questions.
